As of February 20, 2020, I will be closing my neurology practice. It has been a great privilege to know you and to participate in your healthcare.

I understand that finding a new doctor can be difficult. Please make arrangements for a neurology follow-up in the near future.

Below this letter you will find the contact information for several neurologists in your community. You may also wish to contact your primary physician for a neurology referral.

To ensure proper follow-up and continuity of care, please request that all new x-ray reports, lab test results, and specialist letters be forwarded to your new doctor rather than to me.

Accompanying this letter you can also find the contact information for the facility where your medical records will be stored. They will provide you with instructions and a form for the release of medical records.
